 Family Group Submissions

Welcome to the Family Group Sheet Data Entry Area!

Please read the instructions below, then click on the link to open the family group sheet form. Note:  Adobe Reader ©, is required to view the form.

Once you enter your family information and submit the form, it will be sent to the site administrator via email and then will be reviewed. Upon acceptance, all of your family information will be included in the website and you will then be able to see your direct relationships to each person you view.


  1. Fill out the form below (including the Red boxed mandatory fields).

    • Please fill out the form as completely and accurately as possible.

    • The left margin of the form includes tips for formatting.

    • After filling out the form for your own family, please submit additional forms for your parents, married children, grandchildren, etc. if you are up to it.

  2. Select "Submit by Email"

    • If you use an email program such as Outlook© or Eudora©, your email program will start and automatically attach the filled out form to a new email and addressed to me.

    • If you use web based email such as Gmail© or Hotmail©, the form will prompt you to save the form and give you the email address to send it to.

    • Enter the email address provided and attach the completed form.

  3. Hit send and you are done!
